Taking Action to Reduce Chronic Disease
Chronic diseases are a prevailing and costly health concern in the United States. The problem is progressively increasing, with over half of all Americans experiencing at least one chronic condition. According to the Centers for Disease Control and Prevention (CDC), approximately 130 million adults in the US are living with some form of chronic illness, and these figures continue to rise.
Chronic disease represents a significant public health concern in the United States. In Collier County, Core Health Partners, in collaboration with community partners, are dedicated to mitigating its impact on individuals’ lives through the Togetherhood initiative. This initiative aims to enhance healthcare accessibility and promote healthier lifestyles for all residents.
The Togetherhood initiative encompasses valuable resources, including nutritional counseling, physical activity programs, and improved access to preventive care. Furthermore, it prioritizes the dissemination of knowledge on the importance of leading a healthy lifestyle and strives to foster strong relationships among healthcare providers, patients, and community members. By doing so, it aims to facilitate better health outcomes.
Core Health Partners specializes in chronic disease prevention, testing, and self-management. It is recognized for delivering the American Diabetes Association’s Self-Management Education and Training Program and is also certified to administer the Autism Diagnostic Observation Scheduled (ADOS) test. These services are provided in community settings and are available in English, Spanish, and Creole languages.
The exemplary standard set by the Togetherhood initiative serves as a blueprint for local partners aiming to effect tangible change within their communities. By pooling their resources and expertise, Core Health Partners makes a significant positive impact on the lives of those affected by chronic disease in Collier County. This community outreach can also serve as an inspiration for similar efforts in other locations, fostering healthier environments nationwide.
To ensure the thriving of our communities, it is crucial that we collectively commit to investing in our health and well-being. With the appropriate resources and education, we have the capability to collaboratively alleviate the burden of chronic disease for all.

Getting started is easy! You can self-refer by filling out the form on our website (use button below) or by calling our Immokalee office to schedule an appointment. Our program is covered by most insurances, Medicaid, and Medicare, and we also offer a sliding fee discount for eligible patients. By taking the first step towards self-management, you can take control of your diabetes and live a healthier, happier life.
