How Can You Get Started?
Getting started with our Diabetes Self-Management and Education program is easy. You can self-refer by filling out the form on our website (use button below) or by calling our Immokalee office to schedule an appointment. Our program is covered by most insurances, Medicaid, and Medicare, and we also offer a sliding fee discount for eligible patients. By taking the first step towards self-management, you can take control of your diabetes and live a healthier, happier life.
