Togetherhood Initiative's Diabetes Education Program Earns Recognition from American Diabetes Association

 

Core Health Partners' diabetes self-management education program, available at Togetherhood Initiative program sites in Collier County, has received continued recognition from the American Diabetes Association (ADA). Originally recognized in July 2019, this program offers high-quality education services to its patients.

 

Since its establishment in 1986, the ADA Education Recognition effort has been a voluntary process that ensures approved education programs meet the National Standards for Diabetes Self-Management Education Programs. Programs that achieve recognition status have a team of knowledgeable health professionals who can provide state-of-the-art information on diabetes management.

 

Self-management education is vital for effective diabetes treatment. Compliance with the National Standards results in greater consistency and quality of education for individuals with diabetes. Participants in an ADA-recognized program receive instruction on self-care skills necessary for better diabetes management. Approved education programs cover various topics, including the diabetes disease process, nutritional management, physical activity, medications, monitoring, detecting and treating acute and chronic complications, goal setting, problem-solving, psychological adjustment, and preconception and gestational care.

 

Ensuring high-quality education for patient self-care is a primary goal of the Education Recognition program. By supporting healthcare teams and increasing knowledge and awareness of diabetes, patients can take a significant role in managing their condition, potentially preventing unnecessary hospital admissions and complications.
